Purpose of event review
A fair event review seeks to understand what happened, why it made sense at the time, what conditions shaped performance, and what response best supports safety, accountability, and organizational trust.
Key review questions
Reviewers should ask: What was the intended action? What actually happened? What were the environmental and system conditions? What did the person know at the time? Was the risk recognized? Was there a pattern of drift or normalization?
Consistency across cases
When similar cases are treated very differently, staff lose confidence in the process. Consistency does not mean identical outcomes in every case, but it does require clear principles and a disciplined review method.
From review to action
A strong review ends with concrete actions such as policy simplification, redesign, coaching, training, communication improvement, or escalation when behavior is truly reckless.
